Summary of - Radiologic Outcomes of Bilateral and Unilateral Perthes Disease: A Comparative Cohort Study

Abstract

Introduction: This study aimed to compare the radiologic outcomes of bilateral and unilateral Perthes disease, as well as to assess the outcomes of synchronous and metachronous bilateral Perthes disease.

Methods: Over the past decade, 353 children with Perthes disease were monitored from initial presentation to healing. Among them, 37 had bilateral involvement (11 synchronous and 26 metachronous onset). The radiologic outcomes of each hip in children with bilateral disease were compared to those of 148 children with unilateral disease, matched for age, sex, and treatment. Both groups were treated with a proximal femoral varus derotation osteotomy if they met the surgical criteria. The primary outcome measure was the shape of the femoral head at healing, assessed using the Sphericity Deviation Score (SDS).

Results: Children with bilateral disease were younger than those with unilateral disease (6.2 vs. 7.03 years; P<0.001) and experienced a longer duration of the disease. All other characteristics between bilateral and unilateral cases were similar. The SDS values for unilateral and bilateral disease were comparable, as were the SDS values for synchronous and metachronous bilateral disease. The impact of early surgery on disease progression in bilateral cases was similar to that observed in unilateral cases. The age of disease onset was the sole factor influencing the SDS in bilateral cases.

Conclusion: The onset age for bilateral disease is lower, and its duration is longer compared to unilateral disease. However, the outcomes of the disease are similar.
